Enrollment Packet 2012-2013

Please select the appropriate link below to download the necessary forms needed to complete the enrollment of your child for the 2012-2013 school year.  The completed forms are due back to school no later than May 15, 2012.  Also, please remember that there will be an important information night for all students on Thursday, May 10th at 7:00 p.m. at school.  We look forward to seeing you there.   Should you have any questions regarding the enrollment packet please don’t hesitate to call the school.
