PE CREDIT FORMS – 9th Grade ONLY

Students are required to have one year of Physical Education to graduate from Albert Einstein Academy.  We recognize that some 9th grade students may qualify to receive their PE credit from playing 3 sports at AEA, an outside sport, or a combination of both.  Forms are available below.

1)  Student plays for 3 teams at AEA:  Form 1 – PE Credit Form, AEA Sport must be filled out and submitted to  Coach Wood.   The Assistant Athletic Director will mark attendance during each season based on the Coach attendance record.  The student/athlete must attend regularly and have fewer than ten non-medical absences during the school year, or they may not be eligible for credit.  If the student/athlete chooses not to be apart of any team, is injured or not able to play on any of the sports specified on the AEA Sports form, the student/athlete will not receive PE credit.

Form 1 – PE Credit Form, AEA Sport

2)  Student plays for an outside sport (practices and game equaling at least 7 hours per week)  and plays for at least 1 AEA sport:  Form 2 – The student/athlete must attend regularly and have fewer than five non-medical absences at practices and games, during the school year, or they may not be eligible for credit.   Please submit both forms: AEA PHYSICAL EDUCATION CREDIT FORM – OUTSIDE SPORT  (monthly)  and  AEA PHYSICAL EDUCATION CREDIT FORM – INTERSCHOLASTIC ATHLETICS must be filled out and submitted to Coach Wood.    If the student/athlete chooses not to be a part of the outside sport, does not play at least 1 AEA sport, is injured or not able to play on any of the sports specified at Albert Einstein Academy, the student/athlete will not receive PE credit.

Form 2 – PE Credit Form, Outside Sport & 1 AEA Sport

3)  Student plays for an outside sport for at least 15 hours per week (ie practices each day, Monday to Friday for 3 hour): Form 3 –  AEA PHYSICAL EDUCATION CREDIT FORM – OUTSIDE SPORT must be filled out and submitted at the end of each month to receive PE credit at the end of the year.  If the student/athlete chooses not to be a part of the outside sport, is injured or not able to play for the outside sport, the student/athlete will not receive PE credit.  Student must turn in the paperwork on time each month, the first school day of the month, or may not receive PE credit.
